We connect with you
After you send your referral form, our friendly support workers will reach out to you to discuss your goals.

Step 3
Welcome meet and greet
Once you’re eligible, we’ll arrange a warm welcome meeting where you’ll meet your new iCare support workers. You’ll also receive a welcome pack and your first schedule of activities.

The first step of this process is to fill in our Service Referral Form. Before filling out this form it’s important to know that you will need a treating professional (someone like a doctor or a specialist) to help you fill in parts of the form. Our service is available to you, if your NDIS plan includes “CORE Supports”.
